Help Moving Into the Community

Many people with disabilities who live in long-term care facilities or other institutional settings would like to live in the community. If you are in this situation, check out Georgia’s Money Follows the Person (MFP) program.

Money Follows the Person is a Medicaid program that helps people transition from facilities into living in their own homes or in the community. MFP can help you:

  • Decide which community-based setting is possible and the right choice for you
  • Find housing and move into your new home, including helping with costs like the security deposit, household items, moving costs, and modifications to make the home accessible
  • Plan your life in the community and set goals
  • Sign up for a Medicaid waiver so you can get the services you need in the community
  • Connect with community-based organizations

Who Qualifies?

You may be eligible for Money Follows the Person services if you:

  • Have Medicaid coverage
  • Have lived in a long-term care facility for at least 60 days, and
  • Qualify for a Medicaid waiver because you need the level of care normally provided in an institution and want to live at home or in the community. Learn more about waiver programs.

Note: Waiver programs often have waiting lists, but there are usually spots saved for people who apply through the Money Follows the Person program, so if you qualify for MFP, you can probably skip the waiting list.

How to Apply for Money Follows the Person

To apply for Money Follows the Person services, contact your local Area Agency on Aging. An options counselor will help you figure out potential resources in your community and connect you with an MFP Transitions Coordinator.
